An organization that fosters hope and opportunities to empower children, youth and families throughout Manitoba requires an experienced Clinical Case Manager (CCM) within the Transition Services team. The CCM is responsible to provide leadership, development, and supervision of case and treatment plans for youth placed in Transition Services. CCMs are also responsible to contribute to the ongoing development of the program.

DUTIES AND RESPONSIBILITIES: of the CCM include, but are not limited to:

Client Planning:

  • Ensures a systems approach to planning
  • Co-screens referrals for admission to Independent Living with Support program (ILS)
  • Attend intake meetings for assigned clients that include the client, social worker and any other significant people in the youth’s life
  • Participates in the placement process by detailing pre-placement, and post-placement plans
  • Develops the care plan through consultation with the youth and ILS team (i.e therapist, Housing Support manager, tenancy Support worker, placing worker)
  • Monitors and re-evaluates the care plan
  • Ensures communication of the care plan to ILS team and client’s social worker
  • Schedules discharge planning meeting and develops discharge summary
  • Supports school registration and liaises with school personnel and client social worker
  • Ensures appropriate and timely record keeping for individual caseload
  • Participates in advocating for youth and assisting youth with self-advocacy
  • Participates in providing after-hours support to the program through an on-call system

Ongoing Transition Services Development:

  • Attends mandatory agency and program events
  • Record keeper for client mini-meetings
  • Attends team meetings, client meetings and agency strategic planning events
  • Attends training as required
  • Liaises with other agency programs
  • Participates in agency committees as approved by the Coordinator
  • Represents the agency and Transition Services at agency and community functions
  • Other duties as assigned

HOURS OF WORK: 35 hours per week, Monday-Friday from 8:30 a.m. – 4:30 p.m.

ESSENTIAL REQUIREMENTS:

  • Post-secondary degree in human service field with 3 years’ experience in one or more of the following areas: child welfare; residential care; case management.
  • Valid Driver’s License
  • Willingness to obtain training as per agency policy

PREFERRED REQUIREMENTS:

  • Bachelor of Social Work degree
  • Knowledge of systems for children in care, school, family functioning and child welfare
  • First Aid/CPR, NVCI, ASIST (or comparable), Child Development Training, or willingness to obtain
  • Knowledge of Indigenous languages and cultures would be considered an asset
